Our CORE Case Management Program is a high-touch, deeply personalized service built around the real-world challenges facing those in early recovery. While we retain the capacity to manage high-acuity psychiatric cases, substance use recovery, and complex behavioral issues, our true specialty lies in helping clients build a life worth living from the ground up.
We do this by integrating structure, strategy, and a deeply clinical lens with personal connection and purpose-driven support. Every plan is individualized. Every outcome is client-centered. Our goal is to not only stabilize clients but to activate them—to help them take ownership, define their goals, build skills, and ultimately carry the weight of their own lives.
Unlike many "coaching" or standard case management models, our team includes clinically trained and licensed professionals at the master's level. They bring lived experience, professional rigor, and evidence-informed approaches into the trenches of each client's life.

Our structured approach ensures comprehensive support from initial contact through long-term recovery.
A complimentary phone assessment is conducted to determine basic background, presenting concerns, and program fit.
If aligned, we schedule a full clinical intake.
Includes comprehensive biopsychosocial assessment, review of history, risk screening, family input, and goal exploration.
Billed separately; partially credited if case management proceeds.
In the first seven days, we co-build a functional roadmap with the client and their support system.
Week 1 intensive planning period.
Each client is assigned a dedicated case manager who serves as the consistent point of contact across all domains of care.
Continuous support and coordination.
Minimum of two hours, along with daily text, phone, or virtual check-ins
Schedule-building, habit tracking, social navigation, and logistical execution
Regular coordination with treatment providers, psychiatrists, coaches, schools, and employers
Weekly updates for stakeholders with proper consents and monthly clinical progress reports
Structured involvement as outlined in care agreements
That depends on the case. We're present when needed whether that's at court, at doctor's appointments, on the phone with a landlord, or helping manage a financial slip-up. Our team walks alongside the client to reinforce responsibility, not remove it.
At CHC, we view family involvement not as optional but as fundamental. Whether the family is actively engaged or estranged, functional or fractured, it remains the primary relational ecosystem for most clients.
As such, we directly engage families as co-architects in the process. This may involve mediation, psychoeducation, boundary-setting, crisis de-escalation, or collaborative goal tracking. Our case managers help recalibrate family systems to support not sabotage progress.
This work is structured, contract-bound, and clinically informed.

Our service is grounded in evidence-informed principles of Mindfulness Based Cognitive Behavioral approaches (MCBT). Mindfulness-based cognitive behavioral strategies are delivered by licensed clinicians or incorporated into care planning in coordination with outside providers. This is structured, intentional, outcomes-driven case management for clients who need help becoming functional, independent, and self-sustaining. We are not here to do life for our clients, we're here to teach them how to carry it.
When families are tired, clients are stuck, and there's disconnect in care and continuity, this is where we come in. A surviving attachment to rely on and support growth. MCBT principles, and intent, is what anchors every layer of our work.
Clients don't just talk about goals they learn how to define them, challenge the thoughts and behaviors that block them, and take measurable action. We reinforce this through weekly planning, daily accountability, and consistent reflection.
Every interaction is an opportunity to realign with what they want and interrupt the cycles that have kept them stuck. It's not just support it's structured, intentional change work, and it's the reason this model actually moves people forward.
