The CORE Case Management Program is a high-touch, deeply personalized service built around the real-world challenges facing those in early recovery. While the team retains the capacity to manage high-acuity psychiatric cases, substance use recovery, and complex behavioral issues, the true specialty lies in helping clients build a life worth living from the ground up.
This is accomplished by integrating structure, strategy, and a deeply clinical lens with personal connection and purpose-driven support. Every plan is individualized. Every outcome is client-centered. The goal is to not only stabilize clients but to activate them: to help them take ownership, define their goals, build skills, and ultimately carry the weight of their own lives.
Unlike many "coaching" or standard case management models, our team includes clinically trained and licensed professionals at the master's level. They bring lived experience, professional rigor, and evidence-informed approaches into the trenches of each client's life.

Central point of contact across all providers. We coordinate with therapists, psychiatrists, coaches, schools, employers, and legal counsel as needed.
The structured approach ensures comprehensive support from initial contact through sustained progress. Every phase is adaptive to the clinical picture.
A complimentary phone assessment to determine basic background, presenting concerns, and program fit. If aligned, we schedule a full clinical intake.
Biopsychosocial assessment, review of history, risk screening, family input, and goal exploration. Billed separately; partially credited if case management proceeds.
In the first seven days, a functional roadmap is co-built with all relevant parties. Core goals, structure, clinical integration, and action steps.
Each client is assigned a dedicated case manager who serves as the consistent point of contact across all domains of care. Continuous support and coordination.
Minimum of two hours, along with daily text, phone, or virtual check-ins.
Schedule-building, habit tracking, social navigation, and logistical execution.
Regular coordination with treatment providers, psychiatrists, coaches, schools, and employers.
Weekly updates for stakeholders with proper consents and monthly clinical progress reports.
Structured involvement as outlined in care agreements. The broader support system is integrated as co-architects of the process.
24/7 availability for crisis de-escalation, safety planning, and emergency coordination.
Recovery does not happen in isolation. The relational ecosystem — whether actively engaged or estranged, functional or strained — shapes outcomes. Systems-level work addresses the dynamics that either reinforce or undermine progress.
This may involve mediation, psychoeducation, boundary-setting, crisis de-escalation, or collaborative goal tracking. The aim is to recalibrate the surrounding system so that it supports rather than undermines clinical progress.
This work is structured, contract-bound, and clinically informed. When more intensive daily support is needed, Companion Services complement case management with structured, on-the-ground oversight.
